Introduction

Palliative Medicines in Community Pharmacies (PMCP) Project

Thank you for your interest in the Palliative Medicines in Community Pharmacies (PMCP) project Grant Opportunity 2026.

The Health and Community Services Directorate (HCSD) is seeking to engage a community organisation to provide project management for the delivery of the Palliative Medicines in Community Pharmacies (PMCP) Project in the ACT.

The PMCP aims to strengthen community pharmacy capability in palliative care through a range of initiatives, including improved access to palliative medicines, enhanced collaboration with specialist palliative care prescribers, and targeted upskilling in the palliative approach.
The PMCP Project will engage up to seven (7) community pharmacies to stock core palliative care medicines. The program will also strengthen collaboration across specialist palliative care prescribers and community pharmacies and extend palliative care medicine availability outside business hours.

To do this effectively, the successful applicant will provide project management for the delivery of the PMCP Project in the ACT for 4 years. The successful applicant will have experience working with primary health providers, managing communities of practice, and an understanding of the palliative approach.

Please read the PMCP Grant Requirements  to ensure you are aware of all the grant process requirements before commencing with your application.

Please see link attached to the draft Deed Template that will be used for the successful applicant.

Please contact the AEOLPolicy@act.gov.au if you have any questions regarding this grant opportunity.
